• The Penalty Charge Notice (PCN) number.
• The vehicle registration number on the Penalty Charge Notice (PCN).
• A statement of reasons as to why you are challenging.
• Evidence to support your challenge (where applicable).
• Email address to receive acknowledgement and a response (optional).
• Contact details.

The Penalty Charge Notice number and vehicle registration number can be found on the front of the Penalty Charge Notice or correspondence you have received.
